Case Manager

The Case Manager provides emotional support, guidance, and services to families dealing with serious medical conditions.  The Case Manager assesses clients and their families and develops comprehensive, individual solutions to meet their practical, emotional, and social needs.  The Case Manager interfaces with children, their parents and siblings, and medical and social service professionals, all in an effort to bring comfort and support to families and joy and hope to their children.

RESPONSIBILITIES INCLUDE: 

  • Assess clients and their families and develop comprehensive, individual solutions to meet their emotional and social needs
  • Interface with children, their parents, and siblings, medical and social service professionals
  • Provide emotional support and guidance to client families, including the parents, patients, siblings, and extended family
  • Visit clients in the hospital and in their homes
  • Provide clients with referrals to other resources both locally and nationally
  • Work with staff, both locally and nationally, to coordinate and implement Chai Lifeline services, including but not limited to food deliveries, family/client events, volunteers, Big Brothers/Sisters, Camp Simcha programs, Insurance Support, medically supervised trips, Family Weekend Retreats
  • Provide advocacy to client families as needed, including making phone calls, writing letters, or accompanying clients to meetings
  • Research local and national organizations to identify resources to meet the specific needs of families
  • Create, organize, and implement programs and events for clients and families
  • Assist in ad hoc assignments

Education: 

Master’s level mental health professional (MSW, MFT) with experience in medical social work or working within a health-related setting

Work Requirmenet: 

Driver’s license and vehicle

Professional Skills: 

  • Must be self-motivated and driven to help
  • Proven experience working with children, youth, and families
  • Excellent written and oral communication skills
  • Ability to multitask, work independently and as part of a team
  • Creativity and initiative needed to develop new program ideas in response to needs
  • Proficient computer skills
  • Nonprofit experience is a plus

SCHEDULE:   

  • Full Time; must be available to work some evenings and Sundays, as needed
