Case Manager

Chai Lifeline is seeking a compassionate and organized Case Manager to support the Southeast Region. The Case Manager works with the client to assess their strengths and needs, plan a course of action, link the client to services, counsel the client and monitor progress. Must ensure that clients receive appropriate, competent, and professional information and advice, referrals and advocacy services to achieve successful outcomes.

RESPONSIBILITIES INCLUDE: 

  • Determine clients' eligibility for assistance
  • Interview patients or their representatives to identify problems relating to care
  • Collect information about clients, maintain and enter information into databases
  • Consult with parents, teachers, and other school personnel to determine causes of problems, such as truancy and misbehavior, and to implement solutions
  • Refer patients to appropriate health care, community or social services or resources, educational or work programs
  • Counsel family members of clients or patients
  • Monitor clients to evaluate the treatment progress
  • Confer with clients to discuss treatment plans or progress
  • Modify treatment plans to accommodate client needs
  • Schedule and confirm appointments for clients, customers, or supervisors
  • Coordinate communication between patients, family members, medical staff, administrative staff, or regulatory agencies
  • Complete documentation required by programs or regulations
  • Maintain case history records and prepare reports
  • Assist in ad hoc assignments

  • Ability to multi-task

EDUCATION:

  • Master’s degree in a mental health, social work, counseling, or related field, or equivalent experience

EXPERIENCE:

  • 3-5 years of relevant work experience preferred

PROFESSIONAL SKILLS:

  • Outstanding communication skills, both verbal and written
  • Excellent organizational skills
  • Excellent time management skills and the ability to multitask 
  • Proficient computer skills
  • Ability/ experience working with sensitive information
  • Nonprofit experience is a plus

SCHEDULE:   

  • Core hours 9:30 am – 2:30 pm Monday-Thursday; evening and weekend hours as needed
