The Case Manager works with the client to assess their strengths and needs, plan a course of action, and link the client to services. Must assure that clients receive appropriate, competent, and professional information and advice, referrals, and advocacy services to achieve successful outcomes.
RESPONSIBILITIES INCLUDE:
- Determine clients’ eligibility for assistance
- Collect information about clients, maintain and enter information into databases
- Maintain case history records and prepare reports
- Research local and national organizations to identify resources to meet the specific needs of families
- Refer patients to appropriate health care, community or social services or resources both locally and nationally
- Provide advocacy to client families as needed, including making phone calls, writing letters, or accompanying clients to meetings
- Work with staff, both locally and nationally, to coordinate and implement Chai Lifeline services, including but not limited to food deliveries, family/client events, and volunteers
- Provide emotional support to clients and family members
- Monitor clients to evaluate progress
- Schedule and confirm appointments for clients
- Coordinate communication between patients, family members, medical staff, administrative staff
Education:
Master’s level mental health professional
Professional Skills:
- Outstanding communication skills, both verbal and written
- Excellent time management skills and the ability to multitask
- Ability/Experience working with sensitive information
- Nonprofit experience is a plus
SCHEDULE:
- Full Time; must be available to work some evenings and Sundays, as needed
