Case Manager

The Case Manager works with the client to assess their strengths and needs, plan a course of action, and link the client to services. Must assure that clients receive appropriate, competent, and professional information and advice, referrals, and advocacy services to achieve successful outcomes.


  • Determine clients’ eligibility for assistance
  • Collect information about clients, maintain and enter information into databases
  • Maintain case history records and prepare reports
  • Research local and national organizations to identify resources to meet the specific needs of families
  • Refer patients to appropriate health care, community or social services or resources both locally and nationally
  • Provide advocacy to client families as needed, including making phone calls, writing letters, or accompanying clients to meetings
  • Work with staff, both locally and nationally, to coordinate and implement Chai Lifeline services, including but not limited to food deliveries, family/client events, and volunteers
  • Provide emotional support to clients and family members
  • Monitor clients to evaluate progress
  • Schedule and confirm appointments for clients
  • Coordinate communication between patients, family members, medical staff, administrative staff


Master’s level mental health professional

Professional Skills: 

  • Outstanding communication skills, both verbal and written
  • Excellent time management skills and the ability to multitask 
  • Ability/Experience working with sensitive information
  • Nonprofit experience is a plus


  • Full Time; must be available to work some evenings and Sundays, as needed
